  • Other 専門看護師(Certified Nurse Specialist：以下CNS)には倫理調整の役割があり，倫理的感受性の高い組織風土を創るための教育的な関わりが求められている．本研究の目的は，がん看護CNS の倫理的感受性のアセスメントの視点と，倫理的感受性を育むアプローチを明らかにすることである．がん看護CNS?名に半構造的インタビューを実施し内容分析法を用い分析した結果，倫理的感受性のアセスメントの視点は，【日常的に丁寧に患者に関わっているか】，【看護師の責任を自覚しているか】，【もやもやする状況を倫理的な問題と関連づけて考えているか】，【事例の事象を倫理に関連づけて説明できるか】，【代弁者として患者・家族の思いを言語化し伝えられるか】，【倫理的な視点で情報を得ているか】，【客観的，俯瞰的に情報を整理しているか】，【倫理的な視点で主体的にカンファレンスをしているか】の8カテゴリであった．また，倫理的感受性を育むためのアプローチは，【倫理的な課題に携わっていることの自覚を促す】，【看護師個々の力を信じ承認する】，【話し合える土壌を作る】，【事象と倫理的課題の関連を示す】，【倫理的な視点がもてるように問いかける】，【多面的，俯瞰的に情報を整理して示す】，【話し合いの仕方についてモデルを示す】の7カテゴリであった．がん看護CNS は，倫理調整の場面から看護師の言動を観察し倫理的感受性をアセスメントすると同時に，倫理的感受性を育むために事象を通して多様なアプローチをしていた．
  • Other This study aimed to clarify the approaches taken by oncology clinical nurse specialists (OCNSs) in assessing and cultivating ethical sensitivity in nurses. Semi-structured interviews of five OCNSs were conducted, and the responses were analyzed by qualitative content analysis techniques. Considerations in assessing ethical sensitivity were classified into the following eight categories: whether the nurse routinely interacts courteously with patients; whether the nurse is aware of his or her responsibilities as a nurse; whether the nurse associates situations of uncertainty with ethical problems; whether the nurse can explain anecdotal events as they relate to ethics; whether the nurse can verbally communicate the feelings of patients and their family members as an advocate for them; whether the nurse takes an ethical viewpoint when receiving information; whether the nurse organizes information objectively and with a global perspective; and whether the nurse proactively speak at in health care conferences while assuming an ethical viewpoint.The approaches to cultivating ethical sensitivity were classified into the following seven categories: encouraging awareness of oneʼs involvement with ethical questions; trust and acknowledging the abilities of individual nurses; creating a foundation for discussion; showing the relationships between events and ethical questions; posing questions to impart an ethical viewpoint; organizing and providing information in a multifaceted manner and with a global perspective; and exhibiting a model of how discussion is conducted.While assessing the level of ethical sensitivity of nurses based on their words and actions, the OCNSs exhibited leadership as facilitators for cultivating ethical sensitivity and adopted a variety of approaches to doing so.
