  • Other [要旨] 本研究の目的は，集中治療室における治療の差し控えや中止に関する意思決定プロセスに焦点を当てて，看護師が終末期高齢患者やその家族の意向を把握しながら，関わる際の特徴を明らかにすることである。終末期高齢患者の治療の差し控えや中止に関する意思決定を支援した経験があるICU看護師８名を対象に半構造化インタビューを用いて質的記述的研究を行った。意思決定プロセスを前，中，後と分けて捉えた看護師の関わりを類似性により分析を行った。意思決定前では，看護師は《現状をどのように理解しているかを家族に確認する》，《患者と家族に働きかける方針を一致させ，チームの連携を取る》など６つの関わりをしていた。意思決定中では，看護師は《家族のわだかまりをなくし，医師・患者・家族の３者間を取り持つ役割を担い，家族が決定できるように寄り添う》など５つの関わりをしていた。意思決定後では，看護師は《家族が心残りのないように，医療職チームで患者と家族に継続性のあるケアを提供し，最期まで支える》など６つの関わりをしていた。《代弁者となった家族へのアプローチに関する困難》は，時期に関係しない関わりであった。ICU看護師は，患者・家族・医療チームの意思決定において調整の役割を担っている。今回の結果から，意思決定における合意形成の過程で，家族に患者のその人らしさを振り返って考えてもらい，本人の希望を擁護するように働きかけることが必要であると示唆された。
  • Other [SUMMARY] PURPOSE: This study aimed to identify ICU nurses’ involvement in the decision-making process for withholding or withdrawing treatment by progressively understanding terminally ill older adults’ and their families’ wishes. METHOD: A descriptive qualitative design was applied using semi-structured interviews of eight ICU nurses who had experience supporting terminally ill older adults in the decision-making process for withholding or withdrawing treatment. The data were analysed within the framework of involvement before, during, and after the process of decision-making. RESULTS: Before decisions were made, ICU nurses were involved in six ways, including ‘making sure that the families understood the patients’ present state’ and ‘cooperating with the medical care team to attain the goal of treatment’. During the decision-making process, nurses were involved in five ways, including ‘conveying the families’ feelings to the patients, eliciting the patients’ wishes from their families, and reconfirming the families’ understanding of the patients’ conditions’. After decisions were made, nurses were involved in five ways, including ‘providing continuous care to patients and their families to ensure that they have no regrets and providing support from the medical care team until the last moment’. ‘Difficulties in the families’ approach to patient advocate’ was not related to the nurses’ timing of involvement. CONCLUSION: ICU nurses play a coordinating role in the decision-making process among the patients, families, and medical care teams. Our results suggest that it is necessary to encourage the families to reflect on the patients’ personhood and advocate their wishes during consensus formation in the decision-making process.
